Learning Lessons and Improving Practice

The Learning Lessons and Improving Practice Sub Group meets quarterly to consider any requests for a case review, but an extraordinary meeting can be arranged following a request from an agency, and with agreement from the Chair.

The types of reviews to be considered by the committee include:
  • Serious Case Reviews for every case where abuse or neglect is known or suspected and either
    a. a child dies; or

    b. a child is seriously harmed and there are concerns about how organisations or professionals worked together to safeguard the child
  • Review of a child protection incident which falls below the threshold for an SCR; and
  • Review or audit of practice in one or more agencies
The Learning Lessons and Improving Practice Sub Group advises Redcar and Cleveland Safeguarding Children Board on processes, procedures and outcomes in relation to conducting case reviews.

The Learning Lessons and Improving Practice Sub Group ensures any lessons from case reviews are shared and appropriately actioned across agencies, as well as lessons from cases that demonstrate good or outstanding practice.
