Target Leaving Care Team

Target Leaving Care Team TARGET leaving care team is part of Adult and Children's Services, Looked After Children Unit and the centre is based in Redcar.

TARGET work with looked after children and care leavers aged 16 to 21 (up to age 25 if still in education or training) whereby each young person has a personal advisor who supports them during their transition to adulthood. There is a drop in facility available for the young people and a variety of groups and activities that promote the important skills needed to live independently.

The young people have a pathway plan detailing their aspirations and reflects each young person's changing needs and goals. The pathway planning process ensures our young people reach their full potential and focuses on the following areas:

  • Health & Development
  • Education, Training & Employment
  • Identity & Culture
  • Relationships & Support
  • Independent Living Skills
  • Accommodation
  • Money
TARGET work closely with partner agencies enabling our young people to access good quality services and advice i.e. housing, benefits, connexions, employment providers, health services, voluntary organisations, colleges and universities.
