The Community Rehabilitation Team is a team of Rehabilitation Assistants, Therapists and a Co-ordinator who offer rehabilitation in your own home.
Who might benefit?
A person who is:
- Being discharged from hospital
- Living at home following a period of illness
What can we assist you to do?
- Become as independent as possible
- Practical care such as washing, bathing and dressing
- Domestic activities
- Shopping
- Pension collection
Referral Criteria
A person must be:
- Resident within Redcar & Cleveland
- 18 years old or over
- Able to gain some independence
- Motivated to regain independence
- In agreement to participate in a period of rehabilitation
- Made aware of any risks involved
- Orientated at most times
The service is available between 8.00 a.m. and 6.30 p.m. seven days a week. The number of visits we make will depend on an individuals circumstances.
Support is offered for up to six weeks, but in some cases, we may need to extend this if further progress is expected within an agreed timescale.
